You cannot affect the existing Unarmed Strike, the best you can do is add a new, more powerful one for the player to use. You wouldn’t be able to do it as a magic item because you can’t add an Action to a magic item at this time. As part of a feature, you would add an Action to that feature to add the new, more powerful unarmed strike. Set it at 4 dice instead of 1, and that’s about as close as you’re gonna get.
I will say that is nowhere close to balanced, it’s essentially a double critical at all times, and on a Crit it’s gonna be bananas.

I’ve been making a ring for my monk character that quadruples the unarmed strike damage. Unfortunately, I have no idea how to do that.
(edit: it’s not for a ring, it’s a feature of my homebrew monk now :)
