Traits

Multiheaded. The underworld tyrant can't be surprised, and it has advantage on saving throws against being knocked unconscious.

Unusual Nature. The underworld tyrant doesn't require air, food, drink, or sleep.

Cold Absorption. Whenever the underworld tyrant is subjected to cold damage, it takes no damage and regains a number of hit points equal to the cold damage dealt.

Undead Fortitude. If damage reduces the underworld tyrant to 0 hit points, it must make a Constitution saving throw with a DC of 5 + the damage taken, unless the damage is radiant or from a critical hit. On a success, the underworld tyrant drops to 1 hit point instead. 

Actions

Multiattack. The underworld tyrant makes three bite attacks.

Bite. Melee Weapon Attack: +14 to hit, reach 5 ft., one target. Hit: 19 (2d10 + 8) piercing damage plus 7 (2d6) cold damage.

Claw. Melee Weapon Attack: +14 to hit, reach 10 ft., one target. Hit: 17 (2d8 + 8) slashing damage plus 7 (2d6) cold damage.

Freezing Breath (Recharge 5–6) The underworld tyrant exhales a 30-foot cone of frigid energy. Each creature in the cone must make a DC 19 Constitution saving throw, taking 42 (12d6) cold damage on a failed save, or half as much damage on a successful one. On a failed save, a creature is also restrained by ice. A creature can make a DC 19 Strength (Athletics) check as an action, freeing itself or a creature within reach from the ice on a success. The ice has AC 17 and 20 hit points, and it is immune to cold, poison, and psychic damage.

Description

At the dawn of the world, there were vast caverns beneath the surface of the material plane, a place known as The Underdark. For hundreds of years, the denizens of the Underdark were able to live their lives, but then came the Fall of Achelis. As retribution for the acts of Achelis' queen, the floating island was sent hurtling down, through the ocean floor, creating the world's largest graveyard as the Underdark was flooded. The tunnels were given a new name: "The Underworld", as the skeletons of its previous denizens littered the ground all throughout. However, death on such a massive scale does not happen without incident. The combined anger and grief caused undead abominations to rise from the remains, the worst of all being the Underworld Tyrant. With three heads and a bone-chilling breath, it stalks the flooded tunnels, eager to share its pain with any it might encounter.
