Undead Fortitude. If damage reduces the puppet to 0 hit points, it must make a Constitution saving throw with a DC of 5 + the damage taken, unless the damage is radiant or from a critical hit. On a success, the puppet drops to 1 hit point instead.

Curse of Rot. Any healing effects/spells will seem to visibly revitalize the Undead Puppet, but will actually deal radiant damage.

Bite. Melee Weapon Attack: +5 to hit, reach 5 ft., one target. Hit: 7 (1d6 + 3) piercing damage plus 7 (1d6) necrotic damage.

Slam. Melee Weapon Attack: +5 to hit, reach 5 ft., one target. Hit: 4 (1d6 + 3) bludgeoning damage.

Scream. (Recharge 2).Summons 1d4 Zombies from outside line of sight.

Description

Undead puppets, mindlessly being controlled by their master. They are nothing more than the head of a corpse (either an animal, monster or playable race) stitched to a seemingly endless rotting/leathery neck. Perhaps its neck was once the body of a huge snake?
