On a Failed Save a Creature becomes Confused for until the end of their next turn and take 12(4d6) Psychic Damage and is pushed back 5ft. On a success the Creature is not confused or pushed back and Takes only half damage.

Permanently Armed. The Assistant is impossible to disarm. As if one tool is removed from their grasp they will just pick up another from their surgery belts

Multiattack. The Assistant makes 2 attacks with it's Medical Instrument and 1 Mind Screech per turn.

Medical Instrument. Melee weapon attack, 5ft Reach, +3 to hit, dealing 5(1d4+3) Piercing Damage and 3(1d6) Necrotic Damage. 

Attack of Oppertunity. The Assistant can choose to use it's reaction to attack a creature leaving their combat range (5ft)
