Amphibious. The kuo-toa can breathe air and water.
Otherworldly Perception. The kuo-toa can sense the presence of any creature within 30 feet of it that is invisible or on the Ethereal Plane. It can pinpoint such a creature that is moving.
Slippery. The kuo-toa has advantage on ability checks and saving throws made to escape a grapple.
Magic Resistance. The Kuo-Toa Psion has advantage on saving throws against spells and other magical effects.
Sunlight Sensitivity. While in sunlight, the kuo-toa has disadvantage on attack rolls, as well as on Wisdom (Perception) checks that rely on sight.
Natural Naivete. The Kuo-Toa Psion has disadvantage on all Wisdom (Insight) checks against enemy Charisma (Deception) rolls.
Innate Spellcasting (Psionics). The Kuo-Toa Psion's innate spellcasting ability is Intelligence, (spell save DC 15, +7 to hit with spell attacks). The Psion can innately cast the following spells, requiring no components:
Cantrips (at will): vicious mockery, mage hand, thaumaturgy, minor illusion
1st level (4 slots): detect magic, shield, mage armor*, magic missile
2nd level (3 slots): enlarge/reduce, mirror image, misty step
3rd level (3 slots): enemies abound, fear, wind wall
4th level (3 slots): compulsion, dimension door
5th level (2 slots): telekinesis, wall of force
6th level (1 slot): otto's irresistible dance, programmed illusion
7th level (1 slot): reverse gravity
8th level (1 slot): antimagic field
9th level (1 slot): imprisonment (Minimus Containment only, dollhouse container, target up to 6 creatures)
*The Kuo-Toa Psion casts these spells on itself before combat.
Psionic Spike. Ranged Weapon Attack: +7 to hit, range 20/60 ft., one target. Hit: 15 (2d10 + 4) psychic damage.
Unarmed Strike. Melee Weapon Attack: +7 to hit, reach 5 ft., one target. Hit: 5 (1d4 + 3) bludgeoning damage.
The Kuo Toa Psion, a godly Kuo Toa that sprung forth from the minds of its lesser kin. Unlike its brethren, its reality warping power is entirely self sufficient and is put to quick use on those who do not play along. Despite its terrifying psionic prowess, it's actually an oddly childish creature that is easily placated with the promise of fun games. Though playing with a Kuo Toa Psion might be fun at first, adventurers quickly discover playtime for this god never ends.
One of the Psion’s most sinister skills is the ability to shrink the party to mere inches tall and imprison them in a miniature faux town or dollhouse complete with puppeteered illusions to keep the fun going. Escaping from this nightmare takes cunning, as the naive Psion, while powerful, can easily be felled by a well-orchestrated deception.

This one has some errors however; for some reason it is listed as "any humanoid" for the creature type and under reactions you put the lore!

Posted Mar 1, 2021I would think that these Kuo-Toa Psions are the result of a failed Ceremorphs (so mind flayers try putting one of their tadpoles into a Kuo-Toa and this was the result) that feed of the nerve activity of nearby creatures
