Thick Hide. The leviathan has natural scaly armor with few flaws - each time a melee or ranged attack is made, roll a d4. On a 1-3, the attack can be rolled taking normal rules into account. On a 4, the attack isn't able to pierce the leviathan's scales, and is negated.
Segmented Monster. The leviathan is 15 feet in diameter, however, it has 15 segments that follow the monster itself. On maps, this is displayed however your Dungeon Master deems appropriate. Normally, this doesn't affect anything - this monster functions the same as any other.
Thrash. The leviathan flails about, striking targets with it's scaly hide. Melee Weapon Attack: +5 to hit, reach 5 ft., 1 target. Hit: 16 (4d4 + 2) bludgeoning damage. Cooldown 2 turns.
Bite. Melee Weapon Attack: +4 to hit, reach 5 ft., 1 target. Hit: 6 (2d4 + 1) piercing damage.
Whirlpool. The leviathan dives underwater for one round, circling around its own body at great speeds. A whirlpool is created, sucking all creatures within a 15 foot radius towards it. Each creature pulled towards the whirlpool rolls a DC 15 strength saving throw - on a successful save, the creature narrowly escapes the clutches of the whirlpool. On a failed save, the creature is sucked into the center and all creatures in the whirlpool are subjected to a Bite attack - see Bite above.
