Shapechanger. The familiar can use its action to polymorph into 1-2 beast forms from the Summon Familiar spell or back into its true form. The forms are chosen by the patron that creates the familiar, and the familiar's statistics are the same in each form except for movement speed(s). Any equipment it is wearing or carrying isn't transformed. It reverts to its true form if it dies.
Deep Sight. Magical darkness doesn't impede the familiar's darkvision.
Magic Resistance. The familiar has advantage on saving throws against spells and other magical effects.
Touch of Madness. Melee Weapon Attack: +5 to hit, reach 5 ft., one target. Hit: 5 (1d4 + 3) cold damage, and the target must make a DC 11 Wisdom saving throw, taking 10 (3d6) psychic damage on a failed save, or half as much damage on a successful one.
Invisibility. The familiar magically turns invisible until it attacks or until its concentration ends (as if concentrating on a spell). Any equipment the familiar wears or carries is invisible with it.
Description
The Great Old One Familiar is a tiny aberration that doesn't belong in this dimension. It was given to (or forced upon) a warlock by their Great Old One through a Pact of the Chain. Each familiar resembles its creator, meaning that regardless of its natural appearance it's horrifying, unnatural, and beyond normal description.
In most situations, it uses its Invisibility to avoid exposing itself to a world that could never understand. It's Shapechanger ability allows it to blend into the world by impersonating local creatures when necessary.
Note: For gameplay purposes this is an Imp. The purpose of this creature is to offer an RP-appropriate familiar for Warlocks with the Great Old One patron that has the same benefits and powers as the Imp. You could think of it as a tiny Cthulhu, a little floating Yog-Sothoth, or a diminutive avatar of whatever other patrons you come up with.

I see this has been added to a bunch of accounts, how cool! I made this for myself when I was playing a GOO warlock and wanted to think through the different slight changes (like it doing cold and psychic damage, and using Deep Speech instead of Infernal).
Hope some of you are enjoying it! If anyone used this I'd love to hear stories about how it played out in your game!
In my game it came off as a real shame. I described it as a brain covered in eyeballs, and roleplayed that I had to keep it hidden because it basically exposed me as linked with an evil entity. The main problem was just that the Imp, and thus this, is too strong! Being able to transform into a spider and be invisible means you are basically invincible and can explore almost any location. I had to actively decline to use it in many circumstances just to avoid annoying the other players.
