- Brute Force Ceremorphosis. The Ogrithid’s attacks are suffused with psionic force. All its melee attacks count as magical for the purpose of overcoming resistance and immunity to nonmagical damage.

Savage Grappler. The Ogrithid has advantage on checks made to initiate or maintain a grapple.
Multiattack. The Ogrithid makes two attacks: one Tentacle and one Slam, or two Slams on one target.
Tentacle. Melee Weapon Attack: +8 to hit, reach 10 ft., one target.
Hit: 14 (2d8+5) bludgeoning damage. If the target is Medium or smaller, it is grappled (escape DC 17). Until this grapple ends, the Ogrithid can’t use this tentacle on another target.
Slam. Melee Weapon Attack: +8 to hit, reach 5 ft., one target.
Hit: 18 (2d10+5) bludgeoning damage.
Brain Rip. If the Ogrithid starts its turn with a creature grappled and that creature is at 0 hit points, the Ogrithid can devour its brain as a bonus action. The creature dies instantly, and the Ogrithid regains 20 hit points.
Savage Charge (Recharge 5–6). The Ogrithid moves up to 20 ft. in a straight line and makes a Slam attack at the end of this movement. On a hit, the target must succeed on a DC 17 Strength saving throw or be knocked prone.
Psionic Rage (2/Day). The Ogrithid unleashes its aberrant fury, entering a rage-like state for 1 minute:

Description
When ceremorphosis grips an orc, the result is not always a refined illithid but can often become an Ogrithid — a nightmare of muscle and fury. Where normal mind flayers glide with eerie grace, an Ogrithid stomps forward like a siege engine, its massive arms dragging knuckles across the stone.
An Ogrithid’s face is a grotesque hybrid of orc tusks and writhing tentacles, its skull stretched wide and low. Its tentacles twitch constantly, dripping with near caustic slime, while its eyes burn with an animal hunger closer to beast than intellect.
Psionic Rage
When an Ogrithid unleashes its Rage, its body swells monstrously. Veins bulge and throb with psionic light, and the thick muscle across its arms and chest bloats with sudden growth. Jagged, spike-like protrusions tear through its skin at the elbows, shoulders, and knees, hardening into bony weapons. Its tentacles thrash violently, and every step leaves cracks in the ground as if the earth itself recoils from its presence.
