Blood Curdle. Melee Weapon Attack: +5 to hit, reach 5 ft., one creature. Hit: 5 (1d4 + 3) piercing damage. If the target is a humanoid, it must succeed on a DC 14 Constitution saving throw or be infected with a disease—Bloodphage.
While infected, the host starts to feel unwell and its speed is halved. When the host's blood is spilled (by receiving piercing or slashing damage), the spray of infected blood gives birth to 1d4 Clot Flies that spawn 5 feet from the host. This effect triggers only once per round.
Description
Illness was once seen as a supernatural affliction of the body. Any symptoms of which were merely manifestations of the unseen, weaving their evil work through the veins of mortality. But what if these pathogens actually emerged in dramatic magical ways we once imagined. Arising from these dark threads is the Clot Fly, a sickening fiend of pestilence that spreads through the blood of its victims.
Using its sharp proboscis it stabs into a warm bodied creature and leaves behind a parting gift even if its extinguished. The victim, suddenly overwhelmed with nausea, becomes a carrier for the Clot Fly to spread. Should the host’s blood meet the air while infected, the spray will form into a new Clot Fly.
Imagine if you will fair dungeon master the nightmarish scenario of an infected ogre slashed deftly by a fighter, only to have the wound spawn a new friendly face to exterminate. How dreadful indeed.

Posted Apr 26, 2020This creature stuck with me so much that I'm considering using it in my homebrew campaign. I'm hoping my adventures can learn to use it as a weapon instead of it strictly being a illness.

Posted Jun 24, 2020I love this idea. I was watching your video and when you started the story for it I immediately started thinking of the next section of campaign to write, which takes place on a pirate ship of sorts.

Posted Nov 9, 2020I made use of these in this evening's game. Players were suitably grossed out. It's a really nice mechanic, good work!

Posted Dec 15, 2020Could you tell me why the statblock's alignement is different from the video's? This is not the only monster to which it happens, too.
