Innate Spellcasting. Ylleshka's spellcasting ability is Charisma (spell save DC 23). Ylleshka can innately cast the following spells, requiring no material components:

At will: telekinesis, teleport, project image

3/day each: blade barrier, fly, heal (self only)

1/day: summon greater demon (as a 9th level spell)

Legendary Resistance (3/Day). If Ylleshka fails a saving throw, she can choose to succeed instead.

Magic Resistance. Ylleshka has advantage on saving throws against spells and other magical effects.

Magic Weapons. Ylleshka's weapon attacks are magical.

Reactive. Ylleshka can take one reaction on every turn in a combat.

Two Heads. Ylleshka has advantage on saving throws against being blinded, deafened, stunned, and knocked unconscious.

Actions

Multiattack. Ylleshka makes thirteen attacks: twelve with her longswords and one with her tail.

Greatsword. Melee Weapon Attack: +17 to hit, reach 10 ft., one target. Hit: 19 (2d8 + 10) slashing damage.

Tail. Melee Weapon Attack: +17 to hit, reach 15 ft., one creature. Hit: 21 (2d10 + 10) bludgeoning damage. If the target is Medium or smaller, it is grappled (escape DC 25). Until this grapple ends, the target is restrained, Ylleshka can automatically hit the target with her tail, and Ylleshka can't make tail attacks against other targets.

Teleport. Ylleshka magically teleports, along with any equipment she is wearing or carrying, up to 120 feet to an unoccupied space she can see.

Summon Demon (1/Day). Ylleshka chooses what to summon and attempts a magical summoning.

Ylleshka has a 75 percent chance of summoning 1d6 vrocks, 1d4 hezrous, 1d3 glabrezus, 1d2 nalfeshnees, or one marilith.

A summoned demon appears in an unoccupied space within 60 feet of its summoner, acts as an ally of its summoner, and can’t summon other demons. It remains for 1 minute, until it or its summoner dies, or until its summoner dismisses it as an action.

Reactions

Parry. Ylleshka adds 7 to its AC against one melee attack that would hit her. To do so, Ylleshka must see the attacker and be wielding a melee weapon.

Legendary Actions

Ylleshka can take 3 legendary actions, choosing from the options below. Only one legendary action option can be used at a time and only at the end of another creature’s turn. Ylleshka regains spent legendary actions at the start of her turn.

Strike. Ylleshka makes a attack with either her tail or with one of her greatswords.

Teleport (Costs 2 Actions). Ylleshka uses her teleport action.

Description

Ylleshka was formed from the souls of two sisters born on a world far from Golarion, where they ruled a nation alongside each other. They were extremely close to each other, often speaking in unison and appearing almost to be a single mind in two bodies, but were cruel rulers and deeply disliked because each viewed the other as her only equal and the rest of their nation as something to be toyed with for their amusement. Their subjects eventually rose in rebellion and ultimately sacrificed themselves through the desperate act of a priest of a now-dead god of freedom who caused the volcano above the twins' capital city to erupt, immolating the rebels and the sisters alike.

The sisters' souls swiftly sank into the Abyss, where their intense closeness in life and the shared nature of their sins caused a unique transformation—rather than each soul forming into a demon of its own, they fused together into a single twin-bodied marilith with two heads and twelve arms.

Ylleshka served numerous demon lords following her genesis in the Abyss, including Abraxas, Aldinach, Areshkagal, Gogunta, Haagenti, Mestama, Nurgal, Orcus, Shax, Socothbenoth, and Zura, later leaving each one's service after betraying them. She also served almost two dozen nascent demon lords, most of whom are now dead—often by Ylleshka's own hand. She eventually entered the service of Baphomet, whom she currently serves as the chief warden of his Ineluctable Prison.
