Traits

Tunneler. The worm can burrow through solid rock at full speed, it can feel the tremors of movement and can target its prey from all sides within range. This uses its burrowing speed.

Regrowth: At the start of the creatures turn, if the creature is currently latched onto a creature, its regains (1d10 + 5) health and that creatures takes half this in damage as its blood is being drained.

Actions

Bite. Melee Weapon Attack: +10 to hit, reach 5 ft., one target. Hit:  (2d6 + 3) piercing damage. The target must succeed a Strength saving throw of DC 15, on a fail the creature is latched onto the target, the target must then make a Constitution saving throw DC16, on a failed save, the creature falls asleep due to the poisonous toxin and takes (2d8 + 6) poison damge. On a successful save, the worm is still latched on but the target doesn't fall asleep or take additional poison damge. (If the worm takes damage equal to 20 in a single round of combat by a creature while latched onto its targeted creature, the worm loses hold.)

Multi attack (2) - Mandible Bite. Melee Weapon Attack: +8 to hit, reach 5 ft., one target. Hit:  (1d8 + 2) piercing damage.

Venomous spit. Ranged Weapon Attack: +8 to hit, reach 20 ft., one creature. Hit: 19 (2d6 + 2) acid damage, the creature must make a Con saving throw DC14, on a failed save the creature takes a further (1d4 + 4) acid damage and falls asleep.

Feed. If there is a creature in range that is asleep the worm may feed, the creature takes (2d8 + 6) piercing damage. The creature regenerates health equal to the damage dealt.

Description

These cavern dwelling hunters burrow their way through the underground caves, tunnels and chasms. Feeling for vibrations to target its pray. The red and brown hardened exterior scales, strong and sturdy for underground burrowing, they have fleshy grey underbellies that release a red somewhat translucent slime to help them slide there way through the dark caverns of time. Armed with menacing mandibles capable of chewing through the hard earth and stone around them, one on each side of the mouth and a smaller one on the bottom of the jaw to help grip its prey in place, behind of which there are hundreds of razor shape teeth that go all the way to the back of the throat. These blood sucking hunters are the thing of nightmares. These huge worms use a poisonous toxin to put its weary prey down to sleep and then begin to ravenously tear apart the creatures flesh and suck the creature of its blood. The carnivorous underbelly beasts tend to hunt in packs of 2 to 6 and overwhelm unsuspecting prey, biting down and tear through its preys flesh. A very dangerous beast that all under ground explorers should fear.

Nests of these beasts are intricate tunnels, 5 feet in diameter. Watch your step, they can feel your movements. Coming across a full nest of these creatures could be the last thing you ever do.
