Adding an item to the bag isn't the issue, but somewhere in the coding, its now duplicating items for the Action aspect. Weapons and Armor are being duplicated.
I have linked several images below, showing the code I get and what I am referring to.
I even removed the single Palm Pistol I actually added and it removed it from the Actions, but the others did not.
I can remove everything and start again but if this happens when I do it, well. Boo.
Also, I added pouches and renamed them to organize for a coming game, not sure if that does something or is the issue, but kinda wanna see this work because adding the Bag Options is GREAT.
Can you provide a screenshot of the multiple actions you're seeing? Only one is visible to me when I view the character sheet:
I see you've mentioned that you use both ad blockers and Beyond20, but that this issue also persists in another browser. Are you able to turn off your extensions, or test in Incognito mode, where your extensions will be disabled by default? This issue is very likely caused by one of those extensions, especially those that are intentionally designed to interact with D&D Beyond (such as Beyond20).
It would still be lingering. You can see all "ghost" items on the Character Builder | Equipment page. I suspect that these items wound up with a "null" or such in their container field.
It doesn't look like you can delete them even from the Builder. We'll need to wait until DDB sorts out how to deal with the issue.

There's apparently a fix now - if you click your Inspiration (you can click it again to set it how it was before), then refresh, it should take care of the ghost items.

UPDATE:
I removed all the items from every bag and such, and when checking the Action Tab, the weapons duplications are still there.
Something is clearly broken in the coding.
Are you perhaps running any ad/popup blockers, browser extensions (like Beyond20), or a VPN that may be interfering with DDB?

I do have Ad blockers and Browser Extensions(Beyond20), but no VPNs.
I opened Firefox which has no Ad Blockers Active or the Beyond20 Extension, then tested it there and found the issue still lingering there.
No items on my character but the Actions were filled with Attacks for multiple Swords, Palm Pistols and such.
I had tried to replicate the issue again to see what cause it to appear but so far have been unable to do so.
Post a link to your character sheet (actual URL, not share link), that way a Mod or Staffer (not me) can look into it for you.

Oh.
Well I deleted it, now I realized how stupid that was to delete.
Yeh, keeping it up to actually have the Mods/Staff view it would have been better than.
Well, lets hope I can't repeat the issue/ create the issue again, if I do. Then I will post it here.

UPDATE:
I remade the character from scratch and it happened once more.
The Palm Pistol had been duplicated and I cannot remove it without the same issue appeared.

In the first post, I have images of what I took a screenshot you can see them there.
I sort of fixed it, but it is still popping up once in awhile.
I was able to remove the duplicate by selecting it and untick the Display Attack, that removed it.
Though I do not know if it is still lingering in the background hidden now.
