I'm pretty sure this bug affects all spells granted by features/traits instead of added to list through manage spells and all items that boost spell modifier.

Yep, it's any class spell granted via a feature instead of using Manage Spells. At the least the Wand bonus will apply to non-Warlock spells and those gained from feats.

As for your attunement thing, the option isn't there if the item isn't equipped yet. That's probably what happened.
But even attuned, still doesn't work on bonus spells.

At this point, I'm not sure if we are. I did see the item has a charge option, and the description for the item doesn't indicate anything beyond the bonus spell slot that the charge option would be there for, so I assume that's what it's there for and that's how you track it. That's how I'll be having my player track it. Obviously, it would be preferrable to have the slot show up on the spells page, but, as it is, I wouldn't think of this as broken so much as not user friendly.
The Charge on the Rod item is more for noting that you've used the "regain a warlock spell slot" for that day, since you can't use it again until after a long rest. The player would need to manually tick that box, and then untick one Pact Slot in their Spells section, in order to account for regaining the slot.

Re: the original issue in this thread for Pact Boon spells for Pact of the Tome - this is still an issue in 2023. The manual workaround for customizing the spell to increase the DC still is effective, but disappointing that such a minor issue would still be around after half a decade of flagging for WOTC/DDB.
