That is the implication, because there is no "off-hand" checkbox when equipping weapons, and if you have the feat then don't check the box. Also that is what that box does when equipping a weapon, it should behave the same for custom actions.

If you customize one weapon as Dual Wield it does adjust your AC accordingly and if it’s your “off-hand” weapon it will take the lack of damage bonus into account if needed...
An idea i would appreciate on the character sheet it identification on homebrew items. Knowing what spells or items are homebrew in the side menus would be helpful. Some of my campaigns use homebrew, while others don't. Picking spells and searching items would be easier to do if you could differentiate between homebrew items from non-homebrew.

One thing you can do to identify homebrew versus official content is look at the details to see if there's a line labeled 'Source' versus a line labeled 'Version.' The source will tell you what book it's from, version indicates it's homebrew. You have to be looking at the full details of the spell etc to see it, but it's there, for most things. That's helpful. I wasn't hoping for a giant icon or anything, but a customized item at least has an asterisk. Something in that design would help the flow of the game I think, when it comes to homebrew look up. I would imagine turning on and off wouldn't help with HB spells as a prepared list if the situation was reversed.

If I understand the question correctly, it would.
When you turn HB (or any specific source) for a sheet, then the sheet owner can see HB items (or items from the specified sources) on the "edit panels", e.g. spell selection, feat selection, adding items. When you turn HB off, these will be hidden from those lists.
But what is added to a character sheet remains there even if the source is turned off: I can turn on all sources, create a Warforged Echo Knight with a Cloak of Billowing and some homebrew feats and spells; then turn all sources off, and I'll still be a Warforged Echo Knight with those items and feats and spells - but if I want to add a spell, I cannot choose a homebrew until I turn that source on again.

In 5e you don't "prepare daily", instead you can change your current selection after a long rest - and if you don't, you just keep the same spells prepared. When you cast a spell, you only use up the spell slots, not the spells prepared.
I understand 5e rules. I could have worded it better. I'm just saying that if you are preparing spells after a long rest, HB would need to be switched on in order to allow HB spells to be accessible.

That could go for any free text fields, including Appearance, or anything in the Notes section - these only have an indicator to add values when you haven't ever edited them - as soon as you save any values there, there is no edit indicator, even if you empty the whole field.

They are already in a centralised place, one reserved for the campaigns. Adding them to the my characters would be more frustrating because the expectation is that on a page called "MY characters" I see MY characters and MY characters only.

The biggest limitation and easiest fix of player character sheet pdf imho is that the "all actions list" is truncated in a small box on the first page. However all these actions are the most important for quick reference in a battle and the best at averting, the dreaded "damn I forgot to XYZ on my turn" frustrations!
In the pdf we often hardly even get into the possible bonus actions before all the info is truncated:
Can we PLEASE, PLEASE, PLEASE, PLEASE just parse this info:
PLUS this limited use list (which for some reason is not included in "All")
to overflow into a final all actions box page in the pdf. That way if people don't want the extra info requested here, they can simply not print the extra overflow pages.
I understand that the info is probably shown in the features and traits box, but it is INFINITELY MORE USEFUL broken down as a COLLATED LIST of actions, bonus actions, reactions, special actions and limited use actions.
PLEASE implement this simple overflow info box, as it would help my new players sooooo much by not having to interrogate reams of feature descriptions everytime it is their turn and just need to know what ACTIONS are available to them. That list is REALLY powerful as it combines all the possibilities afforderd by Features and traits, as well as magic items etc. It is GREAT. Everything you need in ONE PLACE, just what we love about DnD beyond :)
The main trouble with the pdf export is that the frames and sizes are fixed and given, you cannot put more ink on it that fits on the A4 document, and you cannot put a scrollable box with tabs on it either. And what works for you (a collated list) wouldn't work for someone who would then would have to grab a book and try to look up each of those actions' description to know what it does and how it works.

We need the ability in the customise action and customise weapon screen to be able to add proficiency in the weapon or attack. At present you can add a flat bonus to attack and/or damage but proficiency would make this so much easier.

You already can.
On sheet on actions tab click Manage Custom
In the drop-down box beneath ADD NEW ACTIONS choose Weapon.
A new custom action appears above, click it then click Edit.
Here you can add all sorts of info like range, damage, and what ability score (Stat) to use, etc. There is a checkbox called Proficient - click this and it will add your proficiency bonus into the attack roll (since proficiency isn't normally added to damage). The checkbox "display as attack" will, if checked, show the attack on your sheet under Attacks.
If you can detail what you need the attack to read as, I can advise you what options to pick and to set as.
