So I'm homebrewing a false set of vampire teeth that you can wear to do a bite attack and gain hit points. I have no idea what that roll should be. Idk if I should make it a finesse weapon (no profiency) and make the damage 1+STR mod or 1d4+STR mod but idk. I plan on making it a recharge, so you can only use the health regain like 3 times per day, regaining 1d3 charges at midnight, but idk how to balance the damage portion
I would think first you'd need to make a successful grapple check. Then once you've got your prey grappled you could use your attack action on your next turn to make one bite attack against the target, using Str mod and Prof bonus. If you hit their AC they take 1d6 + Str mod of piercing damage, and you heal that amount.
I'm not sure charges or a cooldown or anything would be necessary, since you'd have to grapple the target first, which will naturally reduce how often you will be using this. I mean, as attacks go it's not a very powerful attack, so it'd really be more of a thematic RP element. Still though, it's a neat idea!
I think the degree of rarity or the target level of the character would also be important in determining what characteristics it has. If it's Common/L1, then it's going to be designed very differently to a Legendary/L20, where I'd be adding much more in the way of bonuses.
Regardless, your suggestion of zero proficiency and 1+Str or even 1d6+Str is really weak. Unless you're looking to make it a novelty item people just have but not use, then I'd beef it up...by quite a bit. AnzioFaro's suggestions seem good. If you're aiming for later in the game, I'd beef it up even further.

The Dhampir race has a vampire bite available. You could grant that ability from the "vampire teeth" item.

Yea I plan on implementing this fairly early so I took into account what they said and made it 1d8+str as well as implementing proficiency
