Sorry, for the confusion. I'm still trying to figure out the best way to do this :). It's 1d6 fall damage (6) plus an additional 1d6 damage (3) if she failed the save. She saved (15), so she only takes 6 damage.
OOC: No worries, just wanted to clarify. So Serwa took 6 damage, and, she fell when the bridge collapsed and is at the bottom now, correct? Is she prone or did she avoid that with the same success as well?
The creatures turn was #600 - It disengaged and moved down into the crevasse under the bridge.
OOC: Ah, I see, I missed that it was the entirety of that turn. So both the creature and Serwa are down in the crevasse below. How far away is it from where she is positioned?
Serwa is about thirty feet away, though the rubble at the bottom of the crevasse is difficult terrain (It would require a dash to get within range of it). The bottom of the crevasse is also very dim with an unnatural chill.

Movement: Serwa will use 1/2 of her movement to stand from prone, then move her remaining 15 feet towards the creature - which might only be 1 or 2 squares if rounded down for difficult terrain.
As her Action she uses Lay on Hands (not a spell, but a class feature) to heal exactly 6 Hit Points, bringing her to full, and as a Bonus Action she casts Divine Favor, ending holding her Halberd leveled at the enemy creature, Standing 15-20 feet away from it. Serwa is at F or G 14, depending on round up vs round down on 15 feet of movement in difficult terrain.
As the creature scuttles under the bridge, Xianon nimbly ascends the rest of the north face of the crevasse and hazards a glance at Tomora. The dwarven reputation for hardiness is well-deserved, however, and Tomora seems ready for anything. He gives her a brief nod before continuing on, darting around the adjacent column, past the bridge, and clambers back down into the crevasse. (OOC: He'd be at L13. That should be about 45', right?) Realizing that stealth is unlikely to work on a telepathic creature, he tries something else. "Who are the others?" he asks the creature in a conversational tone. "Are they like you? Where are they?" At this point, Xianon can hopefully see Serwa over the creature's shoulder. He continues the verbal distraction, knowing the odds of getting a straight answer are slim. "Are they friends?"
Serwa advances (not mechanically until next round), noticing Xianon and nodding slowly as she closes the distance on the creature from the opposite side, preparing to engage it, between her Polearm and the elf’s martial arts.
Aster would move to a space between the two bridges (h13), raise his hand against the creature and cast Adversity. DC12 Con Save or take 4 damage of the next attacks type that hits it.
A flame blossoms in Tomora's hand and she stands ready to hurl it down at the creature. Her usual cheerful demeanor has turned to anger. "you Foul thing...I'll turn you into a pile of ash, I will."
Xianon - you notice a wooden chest partially concealed beneath the bridge.
Aster - your light exposes more of the room and you notice a solid wooden door set back in the cavern wall to the northwest (E9). You also notice a narrow passage to the southeast (M18).
(Adversity con save: 20) The creature cowers, moving about nervously as the party closes in from all directions, its large green eye moving about anxiously. It hisses at Xianon as he comes close and the voice rambles in your minds. "Foul things. nasty things. The others, yes...(cackle)...they cut, yes. And break, and smash...these would not like them, no. We call them, yes, the others come...and cut and smash and break these...(cackle)...but these are reasonable, yes. nasty things, but clever. They leave us and the others not come...reasonable yes...yes..."
It notices Serwa drawing near and suddenly darts around Xianon (still in range, just on the other side). Its large eye begins to dilate nervously (it is readying an action).

Assuming the difficult terrain was only in the area immediately surrounding the collapsed portion of the bridge, and movement beyond that in the ravine is normal, Serwa moves to K-14, attacks from there, then moves 5 feet back to J-14. This may trigger the enemy's readied action which could render these attacks irrelevant but I will roll them nonetheless, in case they go off.

OOC: It's up to you, then - with difficult terrain she would be one square short of being able to reach the enemy and could get as far as J14, still outside of reach for a reach weapon. If that is the case, she would throw a Javelin instead, then resume holding the polearm, bracing for a Reaction opportunity attack if it closes with her. (I'd rather ensure her move/action is legal based on RAW.)

Yes, Serwa fell twenty feet and is laying prone at the bottom of the crevasse.

Note, if the creature closes to within her 10 foot reach Serwa will use her reaction for an opportunity attack via Polearm Master.
