PS- I added a homebrewed monster to that character’s sheet using the normal method under Extras. If you look you will see a summoned Bone Golem. So clearly homebrew monsters can be placed in Extras. They just cannot be attached to a feat. Bummer.
I guess it’s similar to how a homebrewed feat cannot be attached to anything using the Feat Modifier. And that modifier won’t actually add a feat if used on a Magic Item.

You can always load custom familiars into the Extras section of a character sheet without any feat, feature, trait, or spell doing anything mechanically. As long as your homebrewed monster is in “[Your] Homebrew Collection,” any players in the same DDB campaigns with you can open the Extras tab on their sheet and hit the [MANAGE EXTRAS] button, select “Familiar” from the dropdown, and they should be able to find your homebrewed monster to add to their sheet as a familiar.
