In my opinion the situation is very similar as if someone would move into a dense fog while still within 5ft. That would also not trigger an AoO because you are not leaving the reach but you would still be heavily obscured. In Khazela's situation she is behind full cover (a layer of sand), before she is beyond the 5ft reach, which she didn't actually leave. She remains at 5ft distance but underground. This is just the same as if a flier would be in the cube above (diagonally) another creature, but remain within 5ft (in this case it would still be visible, but I think it shouldn't matter if your above or below a creature as long as you remain within their reach).

(Thanks for the link. I had been looking around for rulings about this myself and didn't find this one. I see the point they make, that one leaves the effective reach when one moves behind full cover. Although there is no such thing as an effective reach in the rules, it does make sense (to me). I feel, however, that the case of a burrower misses one very essential part that is that someone who burrows does not move behind cover (actively), the floor collapses and the cover is forced upon the burrower, which is not necessarily a deliberate move by the burrower– but moving deliberately out of reach is a requirement by Attacks of Opportunities to trigger. Therefore, I don't really agree with Crawford's clarification seeing a number of points that are missed or glossed over (or simply poorly covered by the rules and only present in his mind). Crawford's effectively the creator of the rules and if this is how he envisioned it or wanted it to be, then who am I to judge. The question is spot on and this is his response, thus I stand by it. Personally, I don't care about how you rule, I will simply follow how you tell me to play and adapt to that. The monk is in a favorable situation where this only affects them partly, they can still disengage and attack, thus loosing one attack per round by this ruling.)

(Melee reach doesn't extend past the floor or walls. Was curious on that kind of ruling and did a bit of searching on it. A good example of it is the earth elemental, https://www.sageadvice.eu/does-an-earth-elemental-trigger-an-opportunity-attack-when-it-burrows/ . But if the gladiators agree that is an unfair ruling, then it will no longer be in effect.)
