My Wild-Magic Sorcerer in our Icespire Peak campaign has produced plenty of funny plot-twists thanks to Wild-Magic Surges, but one in our most recent combat deaged her from 20 to 11. Needless to say, she's now even less inclined to visit Neverwinter to show off to the Glassblower's Guild the improvements to her magic and her control of it (was already iffy thanks to the long-awaited "unexpected but inevitable" Second Accidental Fireball...)
The player we considered the main character turns out was evil secretly and was our big bad evil guy . So we basically walked into a empty chamber to see a throne and mural of the players chracter and we all started laughing in shock at the table. ( we won and I got his armor , which was pretty good armor )
One of the characters in my campaign is a very well endowed endowed female bard. The guy playing her plays her as a very raunchy temptress. They were trying to convince an orc band leader to join them to attack another orc band. She started her usual routines seeming to offer more than she was actually willing to follow thru on and the orc leader took her up on the offer she was seeming to make. The guy playing her was very surprised and we all had a good couple of sessions with it before the other warband was defeated. In the end she hired several surviving Allie’s to protect her at which the orc leader took his band and faded away - but they will be back when he feels he has a better chance af getting his reward.
Not sure if it's a plot twist but it ended up totally different than what we all thought.
In a pre-5e campaign with a merciless DM we had been going through several difficult dungeons and Fighter was killed. Paladin (party leader/jerk) decided to drag Fighter (best friend in real life) along everywhere to spend parties booty to pay for resurrection at temple in next city. Group unexpectedly passed through lich's lair on way out of dungeon. Near TPK. After party defeated lich everyone immediately rested and began healing wounds when I (halfling thief) told DM I was going to "hide in shadows" and pillage any/all small magic items from the lich without Paladin knowing because he was taking all our treasure.
DM allowed it and made me roll some legit stealth rolls etc. What I found was a powerful dagger (don't remember what but it was a LOT better than what I had at the time) and Ring of Enemy Detection. The rest was coinage and large items that I couldn't hide on me. Paladin didn't notice but jerk meta gamed because he seen the DM interacting with me. Party took long rest. Thief and Ranger(my friend) slept in offshoot of cave while Paladin(jerk) and Cleric(jerk) slept in lich lair. During night Paladin and Cleric told DM they were going to attack the cave opening where we were sleeping in order to cause cave-in and bury us. But...
New Ring of Enemy Detection wakes thief up. Thief nudges Ranger and then blends into shadows. Thief scores critical hit with backstab with New OP dagger on Paladin and kills him. Ranger trades blows with Cleric and beats him down enough that he relents. Underdogs WIN!!!
Whole party thought the Paladin was going to rule over the party, get his fighter back and continue to bully us. What a turn around. Party of underdogs drags Paladin and Fighter to temple and was willing to resurrect them but they never played again.
@paulbunyip: That paladin's player is toxic. He shows all of the red flags: main character syndrome, metagaming, unwanted PvP, bullying, etc. It's pretty clear to me that he might be a narcissist. I would have kicked him out of the table and blacklisted him in my local D&D community so that others don't have to deal with his toxic behavior. The paladin and cleric were also evil. Depending on their respective oath and deity, the paladin could have become an oathbreaker and the cleric could have lost his powers and be marked as a traitor.

Drunk blacksmith staggers into the tavern, saying that goblins have abducted his beloved Helga.
Our party takes this as their cue to go after her and save her.
Later, we found out that “Helga” was, in fact, an anvil.
In retrospect, him professing his drunken love for…“all five-hundred pounds of her!” …should have been taken for a sign.
