I’m trying to create a magic item that will protect a character from the first 3 points of damage for each attack that hits them, but I’m not finding any way to reflect this on the character sheet. Any ideas on how to do this?
For example, If the character is hit with a mele slashing attack for 5 points of damage, the magic item would negate the first 3 points of damage allowing the character to only incurs two point of damage.

You would write it as plain text and the player would just subtract 3 from he damage every time they get hit.
