Two-Headed. The dog has advantage on Wisdom (Perception) checks and on saving throws against being blinded, charmed, deafened, frightened, stunned, or knocked unconscious.
Multiattack. The dog makes two bite attacks.
Bite. Melee Weapon Attack: +4 to hit, reach 5 ft., one target. Hit: 5 (1d6 + 2) piercing damage. If the target is a creature, it must succeed on a DC 12 Constitution saving throw against disease or become poisoned until the disease is cured. Every 24 hours that elapse, the creature must repeat the saving throw, reducing its hit point maximum by 5 (1d10) on a failure. This reduction lasts until the disease is cured. The creature dies if the disease reduces its hit point maximum to 0.
Description
A death dog is an ugly two-headed hound that roams plains, and deserts. Hate burns in a death dog's heart, and a taste for humanoid flesh drives it to attack travelers and explorers. Death dog saliva carries a foul disease that causes a victim's flesh to slowly rot off the bone.
I'd say cute but... No.
Is it weird that I want one in Real life?
No it would be pretty sweet as long as one could control it. As Long as it obey commands i think it would be sweet.
It is pretty much Cerberus but with two heads.
Cerberus is just the boss version of this thing.

Lesser Restoration to remove the root disease that causes the poisoned condition.
Cerberus Lite. Same great taste, but only two thirds the evil
Smart thinking, Cleric. Now, that’s a touch spell, right? The creature is going to attempt a DEX save against your spell DC.
Sounds like a good bargain to me,
How do you cure the disease again?
(1 of my players has got it, we stopped playing until I found out the cure.
Well no because one of it’s things is watchdog
someone needs to make a list of nonmagical cures for the different diseases inflicted by monsters like this cute little puppy right here
you don't have to make the DEX save if you are willing to be effected by the spell
Good idea for a home brew supplement; get some extra use out of the Medicine skill and Herbalist’s tools.
It's a dumb idea, but I kinda want to have a PC get this as a pet. It might be better for an Evil campaign or session.
