Why doesn't the elephant have a grapple, or any trunk attack? Any include it in their game?
I'm actually happy that it has 3 Int. Because of this, you can cast Awaken on it!
My Elephant is attuned to a Helm of Brilliance and Horn of Valhalla...It's also smarter than my actual PC...
Well, that is no longer entirely accurate. The Warhorse can now benefit from the Sidekick rules, (cr 1/2 and a friend to a member of the party) making it a mini-fighter that scales with the party. The Elephant sadly can not.
The Elephant still reins supreme in the lower levels, but at higher levels the Warhorse can out-tank it (Barding, 20CON&17DEX, Second Wind), out speed it, and probably even out damage it (extra attack, action surge) If you're playing a Cavalier, building for mounted combat, and aiming for the long game, the Warhorse is the optimal option. Elephant if you don't think you'll get past 14th level, or Axe Beak for style points.

Say that I throwed a creature with this stat block and a few small tweaks at a party of 3 at level 3, would they be able to handle it?

An elephant is considered huge?! elephants are big sure, but their not THAT big
for perspective that would mean they are larger than:
an Aboleth, a Pit Fiend, a Plesiosaurus, a Manticore and a Sphinx! (all of those creatures are considered "large")
^ Yes, the average African Bush Elephant is like 8.2 feet to 13 feet tall, how does an Elephant count as Huge? When I believe Huge goes from 16 FT to 32 FT.. But then I am reminded that the Monster Manual Baboon has stats closer to a monkey rather than a true Baboon.. so I am positive that whoever came up with stats for the Beasts, do not know anything about animals.. or how to use google..
im doing a desert campaign and im dreading the moment my players realise "oh wait! there are elephants in africa right? are there elephants in this desert?" and buy a CR 4 elephant befor theyre probably level 5

What!? That would make it smarter than the average person. Other intelligent beasts (like octopi) are fine with 3. A better choice would be to decrease the INT of the ape.

I thought large is 8-10 feet and huge is 10-15 feet
I guess one benefit of an Elephant having 3 intelligence is that you can make it 10 later with the awakening spell
Let me tell you those elephants are planning something, a dwarf can tell.
Equally confusing when you compare it to some of the other large Beasts like the Plesiosaurus, Allosaurus, and the Hadrosaurus. Especially the Hadrosaurus which easily was double its weight.

Yeah, but a horse is also considered large, and an elephant is significantly larger than a horse.
