The weapon "The Ravager" is not an official weapon not on the homebrew list and not in Unearthed Arcana. I am therefore assuming this is a private homebrew item and the likely culprit. Somebody has made the item and either messed up or gave it a very powerful bonus (considering items intended for final tier, to level 20, are supposed to only have +3 for very rare items, this seems like a mistake given it shows as a rare +2 item).
To test if a bug remove anything homebrew and see if it still shows the issue. If it does a mod will have to check your sheet - although given my level 5 barbarian (mountain dwarf) doesn't have this issue it seems unlikely to be a bug. More likely there's a homebrew something affecting it.

I just had a look at the character and I concur, there's nothing wrong with it.
With a 20 strength at 5th level, the character has +8 to hit in melee as default with a weapon they are proficient with.
The character has equipped an axed called, "The Ravager" that is granting +5 to hit bonus. It's a private homebrew, so I can't see full details of it unless someone links the url of the homebrew item here. :)
At 5th level, I would expect +1 weapons at most, to fit the power curve of the game.
It is not public. Link does not work. I am assuming you have published it but it requires approval first.
I have created a level 5 mountain dwarf barbarian with 20 in strength and given it a standard maul (why did you test with a maul anyway, the base item for that ravager isn't a maui). The to hit remains +8. This confirms it cannot be an issue with Barbarian (I tried all paths), the race or the maul. The only base item with 1d12 strength based is a greataxe or lance. So I tested those and nope, no issue.
Again, I recommend removing ALL homebrew from the character and seeing if it still has an issue. If it does, it is a bug and mod/staff will have to check further. If not, it is a homebrew item but a bug, to find out which one add each homebrew one at a time until the issue appears again.

I have found that sometimes, when you create a weapon with +to hit, the bonus gets added to other weapons that the character has equipped, and it gets corrupted on the item that you created. As others have suggested, try removing all homebrew items from the character and re-add the standard weapons to see if the bonus is still adding.

Yes, the Ravenger is the cause. It gives the character a bonus to hit equal to the STR modifier, that is why, even wielding a different weapon, the character gets a +5 to hit. Stowing the Ravenger and all functions correctly.
The correct modifier, I assume, would be Bonus - Magic.
So, my player's barbarian suddenly became very good at hitting stuff: https://imgur.com/a/tje2lab
It looks like the strength modifier is applied twice... Or did I miss something?

It's public now, but if I give a standard maul to the character, it's the same 'to hit' bonus as the magical weapon: https://www.dndbeyond.com/magic-items/151817-the-ravager

Oh, that's what it means.
Thank you! The item has been fixed.
