I was attempting to create the Giant Transformation from Grim Hollow as a homebrew feat that modified my size to "Large" which is a feature of the transformation. In creating a feat there is a modifier called "Size" and a modifier subtype called "Large" but it does nothing to my character's size when he takes that feat.
Am I missing something? Is there another way to do this? Is there no way to do this, and if not what does that modifier even do?
As I wrote this I was still playing with it, but adding the +5 HP as "Bonus" and "Hitpoints" modifier and subtype seem to also have no effect (Max HP does not get any higher).
Temporary effects are not supported by this website.
The size Modifier does nothing, you need to use the Carrying Capacity Modifier.
Correct, the Hit Points Modifier subtype doesn’t do that.

Secondly, the transformations from Grim Hollow are permanent changes so I wouldn't need the health and size to change temporarily. I think I am picking up what you are putting down though and that I should just either make it a permanent aspect of a homebrew race rather than trying to do it through a feat that can be added or removed; am I understanding that correctly?
I am curious as to why they would add modifiers that don't do anything, but I guess I am at the whims of the devs on this one. Is the only way to increase my hitpoints with a feature or trait to just add it manually in my character's edit page?

You could do it as a part of the race or as a feat, that’s entirely up to you. I’m just not familiar with Grim Hollow is all.
The Size mod technically does do something, you just won’t see it anywhere unless you use the Maps on DDB. The HP mod is sposta technically do something, just not that, and they never got it to actually work anyway.
How many HP is it sposta add? You may have to do it directly on the sheet, but maybe not. If it’s a flat +5 then you’ll have to do it directly. If that +5 is based on something else then maybe not.
Unfortunately it is indeed +5 health being added. Sounds like the answer here is to just track/add it manually. I've just been spoiled by DND Beyond so I was really hoping for some automation, but I can make due with just remembering my character is a size larger and manually add the 5 health.
