The rules in Tomb of Annihilation are a little vague in this point: Can the PCs become lost while traveling down river in a canoe? I get that it's possible by rule, but how do you lose a river?
Only speaking from experience, I haven't read the adventure, but smaller rivers aren't always a single course of water. Often there are spots where it widens into small lakes and then exits from multiple points. You can't always be sure to pick the right path.
You can get lost down rivers, but you will always know if you're going downstream or upstream. I would look at where the river goes, and have there be a chance of them going down different paths.

I think the book describes the rivers as almost stagnant with very little current, so perhaps after combat or a distraction it would be possible to be turned around.
