Inspired by the flesh eater courts of Warhammer, I'm wanting to write an adventure from level 5 to about 10 maybe 15 with Ghouls and similar beasts being the central Villains.
For those unfamiliar with the Flesh eater courts its essentially "Ghoul Kings' who are a special off branch of vampires who have gone mad and believe themselves to be royalty rather than hideous monstrosities and have surrounded themselves with ghouls and cannibals.
My question is how can i upgrade the standard Ghoul up to a point that they can be viable foot soldiers against level 5 PCs and what is the best way to make a Ghoul like vampire boss for the higher levels. Any other ideas and suggestions in between are welcome aswell
If you want ghouls to be more like soldiers rather than feral animals, perhaps just an increase in their intelligence score will do. Maybe give them armor?

From remote temples in jungles, swamps, and deserts, the yuan-ti plot to supplant and dominate all other races and to make themselves gods.
I like the idea of some having armour now that you mention it aswell as an increased intelligence, but if it comes to a fight i don't want my PCs to mow them down as they would with a challenge rating 1 monster, i have been juggling the idea of multi attack but i don't know if theres any other better alternatives or additional changes

Sprinkle a few ghasts in. Those make them much more difficult. As for the boss, what's the intended challenge rating?

You could use my greater ghast for . . . well, whatever you could use a CR 3 ghoul/ghast for.
As for upgrading ghouls, why bother? Just have the PCs be set upon by a mob of the things; with enough attacks, someone's going to get paralyzed.
