Hello there, I’m trying to create a weapon, where the damage increases after a successful hit. It starts off as 1d4, then after a successful hit it goes to 1d6, and then so on until 1d12. It resets if the attack misses back to a d4. Problem is that I have no idea how to do this in the create an item. Is this even possible? Or is it just something that my player will have to keep track of?

Your player will have to track it. For more information please see the General FAQ and the Items & Equipment FAQ: (https://www.dndbeyond.com/forums/dungeons-dragons-discussion/homebrew-house-rules/131411-a-homebrewers-how-to-faq).
