Plague knights are a specific sect of royal medical wards, some of the most hardy and tough soldiers are used as walking lethal petri dishes to test the effects of various diseases and ailments. Eventually masters of the sect can release waves of the diseases housed within them as weapons against their foes.

Whether it be a physical mask you wear or an enchantment placed upon your lungs and organs, you are unfazed by the germs and disease of the world around you. Your body becoming a vessel for the afflictions inside of you. You are Immune to the poisoned condition and to disease. You also have resistance to poison damage and you can breathe normally in harmful gasses such as smoke or the gas from a spell like Cloudkill.

Your physical form is rife with different germs and infections, and you can spread them offensively while in combat. On a failure, the creature is afflicted with your chosen disease and counts as a valid target for your Quarantine Enforcement. On a success, the creature is not afflicted but does have disadvantage against the constitution save of your Body of Research feature for one minute. An afflicted creature can repeat the save at the end of each of it's turns. You can use this ability once per long rest.

Seems good to me. However seeing as how its just the condition i would do Con+prof bonus so he can do it more

The ability to inflict poison to one immune should be a bit of a lower level, I would add it to either your 11th or 15th level ability. Or as a minor extra ability at those levels to overcome resistance (11th) and then immunity (15th). but otherwise this seems fine
I agree about upping the uses of Body of Research, but I would do prof or CON times per short rest as fighters are geared a bit more towards "short rest design" for lack of a better term.

This line is a bit confusing only because Cloudkill doesn't impede breathing in any way, and specifically damages you even if you hold your breath or don't need to breathe. I don't recall any rules about smoke affecting breathing either. I'd probably word this differently, allowing you to effectively hold your breath for several minutes or just not needing to breathe at all.

I changed the mask to three times your con mod hold breath. Body of research sped up to short rest. and Lethal virulence damage upped, as well as making it harder to get out of afflictions.
