I see what happened here. The confusion came up because we have 3 actors on the same initiative (16). Monster A went, at Init 16. It could just as well been Whitley or Balm. Hillard went because Hillard is listed right after Monster A. I went because Biri is listed after Hillard. Hillard and I essentially made the same mistake. We skipped the other two 16s, Whitley and Balm, but Whitley and Balm added in their actions afterward, so we're actually all caught up in the round, with Zombie A and Zombie B yet to go.
It would be useful to have a tie-breaker on the initiative score, so we could list them separate. Wisdom Mod perhaps? That would list us -
1. Whitley 2. Balm 3. Monster A (possibly) 4. Hilliard 5. Dame Biri "Brickhouse" Benikanderandyr 6. Zombie A 7. Zombie B
I acted before Hillard in round 2, when I shouldn't have. My bad. I also thought it was Monster A's turn, when I prompted the DM to have Monster A attack. I think DM took that to mean Zombie A, who hadn't acted (because that actually made more sense than what I said.) Balm, you've acted four times, though I can see why this is confusing. I think Monster A and Zombie A have been confused with one another once or more.
I made the chart above into an editable google sheet. I figure after each action we could add our action to the sheet as a way of keeping track. That should keep us from getting confused.

Channel Divinity: Nature's Wrath. The Wight can make either a Dex or Con save to avoid the spectral vines entangling it. (Not to be confused with the Ensnaring Strike spell.) If she's got the movement left she'll square up next to Balm. If Balm gets attacked she'll use her Reaction to give her the benefit of the Protection fighting style; Disadvantage to the attacker.
The spectral tentacles emerge from the floor and walls, starting to tangle around the Wight. They get partway there, and then the creature gives a mighty shrug, bursting the tentacles. You watch them slowly fade away back in to the floor. The creature's eyes burn brighter as they settle on the warlock, dismissing the Dragonborn outright.
The tangled zombie still moans and tries to break free from the spectral bonds. The bonds have a different idea however, and keep a strong hold on the mindless undead.
The last zombie continues to whale on the stout, hitting him again.

Not trying to be petty, but initiative isn’t really so complicated. Why does it keep getting messed up?
Did I miss Hillard's R4 action? In the Excel sheet I see that he acted in R3, and then I was posting the Zombie's action in R4. If they've gone twice, I apologize.
First combat in play by post here, sorry for how rough it's going. I'll try to smooth it out, right now it looks like we're just missing Hillard's R4 action, and that brings R4 to a close, and starts R5, correct?
I shouldn't have gone yet. I did so because I had been listed under Monster A before, and the Wight took an action, so I posted an action. I didn't list the Wight in the right place. If Hillard posts an action we can start R5.
I screwed up that chart, but am now correcting it. I also screwed up twice independent of that on turn order myself. Our init order, assuming our opponents have a standard set of stats, is -
