Using a Fighter as an example:
L1: 10 + Con Mod Hit points.
L2: L1 hit points + 1d10 + Con Modifier.
L3: L2 hit points + 1d10 + Con Modifier.

So at L3, your total hit points are the 10 from first level, + 2d10 for your second and third levels, + 3x your Constitution modifier because it's added for each level.
NOTE: If your Constitution modifier goes up, it increases your hit points for all levels.
So if at L3 your hit points were 20hp = [10+5+6 (for the hit dice) + 9 (3 x Con Mod)], and at L4 you use your ASI to increase your Constitution to 18, your L4 hit points would increase by:
+1d10 (your hit die)
+4 (for your L4 Constitution Modifier)
+3 (because the higher Constitution Modifier applies retroactively to the previous levels.)
There’s also an option of not rolling. You can take the flat number which is just slightly above average — 1 more than half the die. So a d10, you get 6+con every level. If it were a d8, you get 5+con, etc.
Its a bit safer, since you know you won’t get a 1.

I thought you just reroll your 1 Die + Con Mod but ive seen stuff of PCs with 200 or smth at lvl 20??
If im right or wrong please explain because i dont get how a party could kill a creature that does a 20 Hp damage

Using a fighter's d10 hit die and 12~13 con for +1 modifier:
At level 1 you have 11 (10+1) HP.
For 2 plus, you either roll or take the average rounded up add your con mod and add the total to your current max HP. Using the average method that would be 11 (10+1) + 7 (6+1) = 18 max HP.
18 (10+1+6+1) + 7 (6+1) = 25
At level 4 you can take an ASI, let's say you increase con to 14 for +2. This retroactively changes your max HP from previous levels (all those 1s become 2s) so old max becomes 28 (10+2+6+2+6+2) + 8 (6+2) = 36 for new max HP.
38+8=46 (I trust you at following along fine by now).
Let's say you want really good HP so you take the tough feat. That feat gives you 2 HP per level, retroactively like ASIs. So the break down is 10 (level 1 die) + 5×6 (average die from level ups) + 6×2 (con mod times level) + 6×2 (tough feat bonus times level) = 64 HP at level 6.

Yep. But keep in mind every investment you put into HP is an investment not being put into damage or other skills.
An ASI in DEX for example increases initiative, AC*, attack accuracy*, attack damage*, DEX saves (a common save), and stealth rolls all by +1 (*assuming light armor and ranged or finesse weapons).

How does max HP change as you level up??

Like lets say im a lvl 5 fighter, do i:
Roll 1d10+Con Mod or
Roll (equal to my level) 5d10+con mod [each?? Or is that hit dice?]
edit: OR OR WAIT, lets say i roll a perfect 10 on all 5 levels and my con mod is 1, so each level is 11
So it goes like lvl1: 11, lvl2: 22, lvl33, lvl4: 44,
