I once had 2 of my players hugging a zombie and when another player went to hit the zombie they instead hit there friend 15 damage and he was on 27 hp!
Trying to avoid a fight with a bouncer, the party climbed on the roof, removed tiles, dropped into the bad guys bedroom and exited the room into the guarded corridor - leading to a fight with all the bouncers.

Sure! My first 5e campaign. The 3rd level party was pretty much drained of spells and HP while exploring the BBEG's lair (technically BBCNG, somewhat relevant to the story).
BBEG finds the party recovering from their wounds during a rest and drops everyone but the paladin with a fireball. Paladin swears an oath to leave and never return if the BBEG spares the party. BBEG agrees and even casts a mass cure on the dying players as a show of good faith.
This is where the Halfling rogue decides that our battered and bleeding party can "take" the BBEG because she's clearly used up her high level spells. For those of you keeping score at home, that is in fact one 3rd level and one 5th level spell slot. So at the very least, BBEG still has a 4th level fireball available to take out the party.
Instead, when the Halfling attacks the BBEG decides to cast Giant Insect instead and we (almost) all die horrible deaths while paralyzed by Giant Centipede poison.
Moral of the story, just because you can fight something doesn't mean you should and if the DM hits you with a potential TPK and then offers you a way out, maybe consider taking it.
Our "my purpose in life to keep the rest of you safe" Cleric's epilogue as the only survivor still breaks my heart.😭
Just recently a player snorted bone dust as if it were cocaine in the hopes that he would experience the corpse's memories. I had him roll a con. save against disease.
Also, isn't "does silly things" close to the definition of a player character?
my PC's were traveling through their second city and decided they were going to attack the first NPC they saw, to their surprise it was a character that was an "old adventurer" (one of my own characters from my first time playing) and nearly ended up as a TPK
Party member had a bag of magic gems which imbued curses from a huge table I made. During a fight, she threw one at a tree to see what it would do, causing it to turn into the Bagman. She then threw another one at the Bagman, causing it to have an endless string of handkerchiefs coming out of its nose.
She then tryied to cover up the incident (when it ran off into the woods, enraged) as having seen a deer. Later, they found out that the NPC's they were with had sent someone out to catch said deer. They found what was left of him in a hollow in a tree, face locked in an eternal, silent scream.
