The Devil's Chalice appears as an ornate chalice. The primeval magic surrounding this artifact does not allow spells (such as Identify and Detect Magic) lower than 10th level to reveal the nature of this item.
When a Tiefling drinks from The Devil's Chalice, a transformation takes place. The liquid inside the chalice turns to the blood of the Tiefling's ancestral demon lord. The Tiefling's biology changes fully to that of a demon, losing all human biology in the process.
Notes: You can choose from different fiendish stat blocks to fit the character into your game. It is recommended that if a player consumes and wishes to remain in control of that character, they play off of the monster stat block, instead of the original character sheet, as they are technically not that character anymore.
Plot hook: A Tiefling character (player or nonplayer) who wishes to rid themselves of their demonic heritage.
Gerald Lightbringer: A powerful priest of Lathander who has been tracking the chalice using forbidden magics, and teeters on the edge of losing favor with Lathander. He has spent his priesthood researching this artifact and has become an obsession, consuming decades of his life. He has gray hair, in his mid-fifties, and dons ragged and tattered Lathander robes. His demeanor is scatterbrained but his mind focuses when the topic turns to the chalice.
The final encounter for this plotline should end with Gerald using a 10th level spell of unknown origin that instantly charms the party and pulls the demonic force out of the affected Tiefling. The group will fall unconscious and wake in a nearby temple. Gerald Lightbringer is gone, and the affected Tiefling now looks purely human, but retains their demonic power (i.e Hellish Rebuke, etc.) The party has no memory of the Chalice, and when they leave the temple, they can see a fantastic ornate chalice sitting atop a dais. On their way out, a priest hands the party a broach of Lathander (the Gesture of Lathander (artifact)), claiming it was found with them. The group can choose the take the item or leave it.

- Once per day, the first time a character wearing or carrying the broach is critically hit with an attack, the damage is reduced to zero and the character gains temporary hit points equal to their active armor class.
-The character's next attack deals an extra 2d8 radiant damage.
Notes: The DM may choose to reveal what the item is and does at their own discretion using in-game means such the Identify spell.

it’s completely intentional and meant to protect the item from inspection. It’s how it’s survived through the millennia.

There is no such thing as 10th level spells. I think you mean 9th level spells.
