A Kobold has a +4 to hit; how is this possible as it does not possess the Strength? Is the monster's to hit based more on a failure of the system? Was the AC suppose to be 5 point based rather than 10 points?
Kobolds wield a dagger in melee, which is a finesse weapon - allowing the wielder to use their dexterity ability bonus for attack and damage, instead of strength.
Their sling is a ranged weapon and therefore automatically uses dexterity rather than strength.
The natural weapons (bites, stings, claws, tail strikes etc.) of various monsters will use either Str or Dex, and the stat block will not tell you clearly which is used. A Stirge clearly uses Dex, a Black Bear uses Str. For many creatures it is unclear because their Str and Dex mods are equal (a Constrictor Snake for example), but in most cases it really doesn't matter - the creature just has the To Hit bonus and damage bonus listed on its stat block. The only common case where it matters is Ray of Enfeeblement, which targets Str attacks, but you can just give the benefit to the players when there is doubt.

A CR 1/8 monster has a proficiency bonus of +2. As Stormknight stated, it uses its DEX (with +2). So it has an attack bonus of +4.
It's AC is 10+2 = 12
It's HP = 2(1d6 - 1[CON]) = 2(3.5 - 1) = 5

That is helpful but it does not explain a +4 listed in the monsters section.
Why is this not listed as when I go look for the "To Hit" bonus in Search?

sorry, I found it after a check of proficiency. But a Bite is in both Dex and Str bonus?
