If your players are casting a healing or resurrection spell near your counter-spell wielding bad guy do you ever counter spell that spell? Or do you only counter offensive magic?
I mean a smart caster would. CS is kind of a "dick move" in general and this is just one example of it.
This is why I offer my players a counterspell truce at the start of a campaign. If they say yes, CS is not in the game. If they say no, I don't feel bad using it to greatest effect because they asked for it.
Agreed that counterspelling a heal/rez is a dick move and smart enemies would 100% do it. I also think if it's done judiciously, it can highlight just how evil the enemy caster is and motivate the party to defeat him/her.
One of the most memorable moments of the Icewind Dale campaign I played in involved an enemy mage counterspelling the bard's level 4 cure wounds after the paladin had failed a death save. Mage used her reaction to counterspell, then her action immediately following the bard's turn to attack the downed pally for another failed save. The fate that befell the mage after we healed the pally was something to behold. I still hate that mage, and it's been almost two years since it happened, lol.
It depends. Does the enemy with Counterspell recognize that a healing spell is being cast? Or not? I roll an Arcana check to see and if I fail the Arcana check (DC 10 plus the level of the spell) I don't use Counterspell. That makes it slightly random and fair to the PC's.

An ally starts casting a spell on a (fallen) comrade, that's enough reason to counterspell it.

100% ive done it, with the group I play with, it makes it feel more "real" and "gritty" if they cant 100% of the time rely on their spells, and have to spend time actually planning and thinking of creative ways to copy spell effects, or if they have to actually prep

I would just as much as a PC would do to an enemy.
