People who argue with the DM excessively. I don't mean like a friendly debate with the DM, but people who reeeallly argue their point even after a decision been made at the table. And then get grumpy and whiny afterward. It is just a game after all...
Also! PC's that never want to be with the party and always leave the group soon as they get a chance. The lone wolf types.
I feel you, I used to have a player exactly like that
As a DM, the first few pet peeves that come to mind are:
Long pauses. I don't like a lot of "dead air time" in a game.
Me, as a DM, talking too much. If the DM i(ie me) is talking most of the time, I feel there is a problem. The DM describes the scene then should get out of the way.
Indecisive players. Don't take 5 minutes to make a decision on what your character does -- particularly in combat. If you are discussing things (for an out of combat situation) and interacting with other players, that's fine. Just don't have a long pause where no one is doing anything.

"An' things ha' come to a pretty pass, ye ken, if people are going to leave stuff like that aroound where innocent people could accidentally smash the door doon and lever the bars aside and take the big chain off'f the cupboard and pick the lock and drink it!"
When we used to play in-person, people who would constantly roll their dice. I have been guilty of this myself and now that I DM I regret ever doing it.

The only time that bugged me was the guy who would try to keep rolling dice till he got a good roll, and then wanted to use that for his next to-hit, or damage, or what have you.
No, sorry -- you have to roll for real after declaring your attack.

WOTC lies. We know that WOTC lies. WOTC knows that we know that WOTC lies. We know that WOTC knows that we know that WOTC lies. And still they lie.
Because of the above (a paraphrase from Orwell) I no longer post to the forums -- PM me if you need help or anything.
What I hate: players complaining about the boss fight's difficulty AFTER they survive it with over half their hp remaining....
That’s when my snark would bubble over and with an absolutely deadpan expression say something like:
”I agree, that encounter wasn’t well balanced. I’ll have to start giving the monsters more HP or something.” And then walk away, possibly muttering something about more Legendary Actions....
- The player that asks for a specific check before trying to do something.
Not sure if it's exactly the same thing, but I had a problem with a player on friday evening, who told me "I want to do an Insight check"...
I was having a discussion about this a couple weeks ago lol. I think that's the only skill where as a player you actually say the skill you want to use, everything else is I want to do X, ok roll whatever skill the DM decides.
I'm not a fan of rolling to hit then taking forever for damage. Also "Oh it's my turn? Well what should I do." 4 rounds into combat lol.
If an NPC says something to a PC, and the player asks “Can I insight that to see if....?” that doesn’t bother me. I mean, what other check would it be? Honestly?
But otherwise, I generally agree that it’s better to describe what you want and let the DM name the check, or at least ask generally “Is there some kind of check I can make to....”

It’s more just the fact they are assuming. An insight check is pretty straight forward, but some players try to weasel a check just because they are proficient. Some might say it ruins the immersion as well.
sorta related: dm says “make a perception check” Player “you sure it’s not an investigation check?” lol
My pet peeve is players who fail to roleplay their Dump Stat. If you have an 8 in as one of your stats you are measurably below human average, and it should show in more ways than just skill checks.

Player who look up stat block in the middle of an encounter
Doesn't work in my campaign... I mean for run-of-the mill fights with Ankhegs or something it might. But for any major bad guy, it's custom.

Yep this is what I mean. “Can I make an acrobatics check to...” just tell me what you want to do, I’ll tell you the check to make.

Yeah that is a fun one. Just exactly how easy do they expect it to be??

Players that still don't know where to find something on the character sheet after years of playing.
playing since 1986
Players who read the adventure.
Players who gripe when I break a rule to help them out. I allow casting a line of sight spell through ice last session.
People who always late.
Moochers with money.
People who response to this thread that have mention other things that bother me:) Big evil grin.

The players being on their phones during the game, especially when their turn is coming up.
Players not paying attention so they need clarification on the environment anytime their turn happens.
Players not knowing how their character works, despite being told how it works about 20 times.
Skill Dogpiling. NOT EVERYONE GETS TO ROLL FOR PERCEPTION!
