Is it possible to homebrew languages or otherwise edit existing ones to reflect a homebrew world?

Unfortunately, no. It is not currently possible to homebrew languages. However, it is possible to add languages directly to a character sheet, any language you like, even homebrewed languages. See the video below for more info:
I'm currently working on a Fighter subclass that, at 15th level, gives you a reaction very similar in style to the Tempest Cleric's Wrath of the Storm. The main difference (aside from an obviously different power level) is that there are two ways to use it. For simplicity, I'll just put the feature in a spoiler here:
Thunderous Retribution
15th-level Storm Knight feature
You have learned to channel the storm within you to retaliate against attacks. When a creature you can see hits you with a melee attack, you can use your reaction to erupt with thunderous energy. The creature must make a Constitution saving throw. On a failed save, it takes 3d10 thunder damage, and it is knocked back 15 feet away from you. On a successful save, it takes half as much damage and isn't knocked back.
Alternatively, when a creature you can see within 60 feet of you hits you or one of your allies within 30 feet of you with an attack, you can use your reaction to launch a bolt of lightning from yourself at the attacker. The creature must make a Dexterity saving throw. On a failed save, it takes 3d8 lightning damage and is stunned until the start of its next turn. On a successful save, it takes half as much damage and isn't stunned.
The DC for these saving throws equals 8 + your Charisma modifier + your proficiency bonus.
You can use this reaction a number of times equal to your proficiency bonus, and you regain all expended uses when you finish a long rest.
If it's not clear from how it's written, you get [proficiency bonus] uses per LR no matter which way you use it, but with the different damage types and amounts, I have to make it two separate actions. I do not, however, want the character sheet to show a full [proficiency] uses for each "separate" reaction, since that's just not how the feature works. How would you get around this? I could maybe make three reactions - one without damage and with the limited use data, and the two others with the correct damages but no limited use data - but that could be kinda clunky and confusing.
I would either do it the “clunky and confusing” way, or change the feature so I didn’t have to. 🤷♂️
Okay. Thanks; at least I know I didn't miss anything I should have known about. I did consider changing it so that you can use each reaction a number of times equal to your Charisma modifier, but I thought that might be a little unbalanced. I don't know....

Paladin main who spends most of his D&D time worldbuilding or DMing, not Paladin-ing.
I have created a subclass option that gives the character a bonus action. I have troubleshooted for a while but when i go to my character sheet the bonus action keeps duplicating itself. It's listed on repeat 16 times under the bonus action section of my actions tab and 4 times under the description of the feature under the features and traits tab. I have only created that one option so far and I'm not sure how to fix it. I made sure that "is granted" is not checked and there's almost no data in the action except for the snippet, description and the range because its not an attack. It has one limited use data point. I have it set up as one use per short rest, that part is displaying correctly. Please help!!

I also tried Modifier: Bonus -Modifier Subtype: Unarmored Armor Class - Fixed Value: 3. This didn't work either. I went and made a copy of Draconic Bloodline Sorcerer to see how that was set up, copied it exactly, and still could not get it to work. Is it because I'm trying to do this as a class feature and not a racial feature?

There is no way to combine the two rolls, no. You must roll the skill from the Skills tab and then roll the d4 separately. That’s usually most easily done directly from the dice trey:
Create them as a wondrous item. Simply add the following modifiers to it:

how can I take a skill check roll and add 1d4 guidance to its output, even if I make a new action to do so.
This is the guidance part at end but I need the skill check first...
You can make a custom Action directly on your character sheet for the 1d4. Like this:

There is no way to combine the two rolls, no. You must roll the skill from the Skills tab and then roll the d4 separately. That’s usually most easily done directly from the dice trey:

Is there a way to create an item that adds an additional first and second level spell slot?
I tried copying and modifying the book of exalted deeds and deleted some of the modifiers but it didn’t work.
The option is not available under the modifier type drop down unless you copy it from book of exalted deeds.
