Please tell me if you have trouble accessing it or I've made any mistakes. Kaboom979 was a great help with this, and I assure you that any problems with the forms are because I messed something up, not him. Voting ends Saturday, November the 5th and ends at noon (EST). Feel free to give feedback and do other cool stuff during this period.
Maybe for the next Inspirational category theme we should pick something more generic. Not to be rude but making space-related content is a mixed bag because not only are there already rules for it, but besides the obvious Psi Warrior with a sun blade, not much of D&D can be ported directly into space.
That's a good idea, but there hasn't been much success with any of the themes for inspirational. For the entire history of CotFB, inspirational options have gotten 1-2 submissions, sometimes even 0. Honestly, I think it might be a good idea to either remove it and allow things like dungeons, adventures, and so on for DM option, or boarded in the category to include both what's currently inspirational option but also include other things.
Yeah, I probably should have picked a more generic category and that's on me. I was going to say it, but HomebrewMindFlayer said it first: I think Inspirational needs to be expanded, it takes too long to make something for it and I've never seen more than three submissions. I dunno how we could expand it though even if we all agreed, so any ideas would be welcomed.
Yes, two scores (I think balance/mechanics and formatting/lore/presentation if I recall correctly) and a text field for each entry for written feedback.
I updated the forms and they should be working now. Again, I'm so sorry for my mistake. Anyways though, the previous links I gave you now work, but HERE is the link again if you need it. I am moving the day when votings ends back by one since starting the proper forms was delayed until now, so voting ends at noon (EST) on Sunday, November the 6th.

Designer's Notes: Some responses I have put together based on the comments I am seeing in the poll for my entries.
To address the comment on the Knower's Guide to Infernal Control stating: "Some of the Tome invocations say that the true name is erased from your book after using them - considering how limited Warlock invocations are and that you can only switch them out at a level-up they should not be one-use things."
This design choice is intentional. There is nothing that stops you from collecting a creatures True Name more than once, simply that each time you exploit its name to freely empower your spellcasting or give it some other effect it resets it. Take for example the Devastating Resonance invocation. In my opinion it would be overpowered if after using your bonus action to extract its true name you could impose disadvantage on every saving throw the target makes against your spells (including cantrips or invocation-based spells) for the rest of combat (or beyond that if it survives). Thats basically a free metamagic option. On the flip side, take the Fateful Naming invocation option. If your allies allowed you to add their names to your tome, then if their names never get erased you could freely cast Bless on the rest of your party for not only the entire combat, but the rest of the campaign. Part of what make these invocation options interesting is their powerful modification of your spellcasting against certain creatures, but in my opinion if these effects are "always on" against those creatures it tips the scale to being too powerful.
To summarize briefly, targeting the same creature multiple times with the same effect is definitely possible, but the erasure detail exists so that you have to invest at least your bonus action in the middle of combat to ensure you get to achieve the special rider effect to your spellcasting.
To address the comment on the Knower's Guide to Infernal Control stating: "Thorn Whip as a cantrip should not require concentration, especially considering that Warlocks want to use concentration spells to get the most out of their spell slots."
What I wanted to do was add an interesting rider effect to Thorn Whip to allow you to use the "chains" to grapple the target. That being said, this is more powerful than a normal grapple because you do not have to use your action to attempt the grapple in the first place (you can do it as part of hitting with your attack) and you can maintain the grapple against a creature up to 30 feet away. In that way, concentration seemed like a fair tradeoff. That being said, I do agree it could probably function without it.
To address the comment on Lost Notes of Iggwilv stating: "Also I think a tomelock should gain the same benefits as a wizard when adding the pages to their tome, including the spell attack/save DC bonus"
I already did tack on a bit at the end at that states that at the DMs discretion other characters from other classes can potentially add these pages to "spellbooks" the possess and specifically used a Pact of the Tome warlock as an example of such an exception.

I was the one who committed these crimes.. ah... gave that feedback. It was late in the evening, I was very tired, I probably misread something. With your explanation the fact that the true name disappears makes sense now :-)
In regards to the feedback I got so far for my two entries:
Juiblex: Yes, it is a complex statblock. High CR by itself should not be a reason to subtract points though.
I like to run more complex monsters in important battles, I guess that is just a difference in DMing style. For example, I DMed Don't Say Vecna a couple of times already, using a slightly adjusted version of my homebrew Vecna statblock together with 16 (buffed) wights, two death knights, each of them had their own individual spell list (one lightning/thunder themed, the other one fire-themed), and a dracolich serving as Vecna's mount. I also gave Vecna an ability that allowed him to share buff spells cast on himself with his death knights as a legendary action. It was a lot to track, but aside from being exhausted afterwards I had no issues tracking all of that stuff. The fact that I have memorized most important spells and statblocks helps of course as I do not need to flip through books or websites all the time and can keep my focus on the ongoing battle.
Regarding Colossal, I never liked how big monsters still could only hit one single creature with their attacks; while I could replace their attacks with AoEs, Colossal allows me to still have primary targets that take full damage and suffer additional effects, while the damage to others is just collateral damage. Players will learn after the first few hits that staying packed together is not a good idea, which means the amount of rolls goes down by a lot. What would you do, what mechanics would you use?
Regarding the slime tentacles, while that may work against some parties, you have to take into account that Juiblex itself wants to consume them, it wants to either move into melee range or uses Whelm, which pulls them into melee range - unlike dragons and other fast flying thing, Juiblex is a monster melees have a rather easy time with, especially if they find a way past its resistances. Also, a properly prepared party will use freedom of movement, which counters most of what Juiblex can do - and a heroes' feast on top of that helps with the abundance of poison down there in the Slime Pits.
Pact Primeval Warlock: I am glad you like it! Being able to charm someone into hurting fhemselves or others was indeed an oversight which I have fixed already. I do not think that expertise in Deception and Persuasion is too strong though, I think it is fine. Without that expertise, you would have the weird situation that a rogue with barely any Charisma would be better at these skills than someone who is magically related to the pact of all pacts, to the negotiations of all negotiations.
Also, from MotM onwards, official statblocks no longer differentiate between spellcasting and inmate spellcasting, they just have a "spellcasting" action.

With that in mind, there are still some surprisingly powerful options among the CR 10+ monsters. The legacy versions of the Amnizu and Oinoloth can cast Feeblemind with Innate Spellcasting. A Rakshasa can cast Plane Shift and Dominate Person. A Sire of Insanity can cast Mass Suggestion
