Does it only provide bonuses when you're Wild Shaped, or does it just have Wild Shape as an option? Also, does it have any additional AC bonus on top of the normal armor?

An armor set the works the same in both Wildshape and when the Druid isn’t in Wildshape. Like Studded Leather would give both forms an AC of 12+dexterity modifier. No other bonuses.
Hmm. I think I'd say uncommon, maybe with attunement. Something to consider is that many animals have natural armor, which wouldn't work with this item. On a crocodile, for example, the item wouldn't actually change the AC. If it were a constant +2 to AC whenever you're in Wild Shape, it would be a higher rarity, but taking a quick stroll through the list of beasts shows almost all of them either having a +1 or +2 from natural armor or being mostly unused in combat.
I'll note, however, that I'm not so great with things like rarity, CR, and spell levels.

An animated shield is very rare it frees up a hand but requires a bonus actionevery minute so I think what you are after would be similar power if it increases AC by about 2.
If it is able to increase your AC my more than 3 or 4 it would be totally broken.
The shield requires attunement if the armorr does not its power goes up significantly.
It's entirely possible to create armour for animals and for a druid they would just wildshape and have a party member help them into it. However, that comes with the downside the druid would have to do this before they enter any combat. The freedom to enter wildshape in combat and maintain armour is significant. Although given a party wizard/sorc can just mage-armour your beast form is also just as convenient and with a feat, item, or more you can also do this for yourself. So, I'd make it the same rarity as the normal armour, just it needs an attunement slot or something. I would not allow special magic armour versions though, like fire resistant armour or whatever. It would just be normal armour, even +1, +2 or +3, but no other features beyond "can wild shape with it", and needing attunement.

Assuming we're talking about Moon druids here, the incredible HP cushion of WS is balanced largely through the low AC of the available forms which is why this kind of thing doesn't already exist. As such, I'd probably make this more rare than equivalent armor anyone could wear.
That being said, it could be interesting to have a magic item that grants the druid's AC when in Wildshape form with the caveat that WS hitpoints are the druids actual hitpoints, and when they go to zero the druid goes down. This might be a little too harsh so I'd probably add another minor benefit, but it would be another way to approach the issue with a risk/reward mix that can be compelling to a player.

Agreed, but the HP cushion that Moon Druids get is going away in 2024. So, this sort of armor will be needed.
