Since I am currently running CoS, I don't allow UA for balance. I will allow custom homebrew (because I want my players to have fun), but only after discussing it at length with the player if it seems like it will have a significant effect on the game. I try not to allow too much stuff, because I don't want to accidentally turn a difficult campaign into a cake walk and be forced to try and either re-balance encounters or disallow/change something after telling the player they could use it.
I love UA, but there's two things. My players don't actually tend to use UA, no idea why. There's also rare cases, like things that I can tell that are overpowered in an instance. While others don't think they are, I particularly found the wildifre druid, oath of heroism paladin, and especially the college of eloquence bard op.
The way of astral self monk was OP, so were many of the more recent subclasses, so I generally don't allow it unless the specifically ask me and I review it.

Yeah, for me the default is always no, but if a player goes out of her way to ask me about a specific subclass for a specific character, I'll review it and probably allow it if it's not clearly overpowered.
Sure, I even approved a Mystic. I can always tweak things if I feel it necessary. I tell players using UA material that I might end up changing things right off the bat though so if it turns out to be necessary it’s not a big deal.
Ever since WotC turn the WGtE into paid for UA material, then formed the new book's changes on WGtE, I have made the decision to let other do the playtesting and I may buy the resulting material in its finalized form. I will no longer allow the use of UA material in my campaigns.
If I do buy finalized material, it'll be through DDB where I can limit myself to those part I agree with and the rest can rot.
If I am asked to play in someone's campaign that is using UA material I will just turn down the offer.
WotC is being a bunch of yahoos in that they take away further access of UA materials that sites like DDB put the money, effort, and time into making because they want you to BUY a finalized version you may not agree with.
Me, I have finally said enough is enough.

That has generally been my opinion of WotC for over 10 years now. But, that being said, if my players want to play UA, I’m not gonna be the one to poo in their cereal.
I allow it, but I usually do so with the caveat that because UA material is untested and unbalanced (in some cases deliberately as more of a proof of concept) there will probably come a point where I will have to homebrew revisions to it (which isn't a big deal, I homebrewed a subclass for one player and revised the Assassin for another in order to better fit the campaign I'm running).

If changes happen, like with the Ebberon release, I'll give them the option of sticking with the UA or changing to the 'real' version.

Here, have some homebrew classes! Subclasses to? Why not races. Feats, feats as well. I have a lot of magic items. Lastly I got monsters, fun, fun times.
