If you were going to necro anyway, you could have at least done a relevant topic. Technically, a thread is dead 6 months after its last post, so not necro, but still off topic.
This question has been asked a few times and the answer is you can't. There isn't a modifier for it. The modifiers were made for official content, and official content doesn't do this. Some day the homebrew tools will be improved.

Next time please just start a new thread instead of necroing. That said, what you are wanting isn’t possible. The homebrew tools are actually slightly prettied-up (and more limited) versons of the tools the developers use themselves to create official items, and as such only really have options/fields/formulas for mechanics that exist in official items. No official item offers différent attack and damage bonuses in 5e, so the tools are set up to accomodate doing so.
It’s not “necroing” if the topic is relevant and the discussion is constructive. DDB does not kill old threads specifically so they can be brought back up in a constructive way. In my interpretation, asking for new information within an existing and related thread is precisely why old thread don’t get locked. (How related this topic is to this specific thread might be debatable however.)

To be honest, I doubt server space is really a concern for forum posts. It's more an issue of keeping discussions centralized and organized.
Most of my google searches for info pull up threads that are a year or so old. Leaving it in one thread would really help people find the info they're looking for.
I certainly appreciate the response, but there is an issue with this method. It changes the hit roll of *all* my melee weapons. I only want it to affect this individual weapon. It does work with the attunement how I want, however. Thank you for the idea!
You can give the weapon a magic bonus of +2 to hit and to damage.
Then give it a weapon damage penalty, Damage - <Weapon type> = -2
Slap the Radiant +3 on top of that. There doesn't seem to be a way to toggle which things only activate with attunement. *shrug*
The problem with this method is that the "Damage - <Weapon type> = -2" is also not affected by the Attunement, so it appears on the stat sheet even before Attunement, essentially subtracting from my strength bonus.

Oops, sorry. I almost exclusively use this site on mobile, and I only get "Report - Reply - Quote" for my options. But I just tested and I can hit "Quote" on multiple posts and it auto-adds them in the reply box. My bad.

You can do this.
You can give the weapon a magic bonus of +2 to hit and to damage.

HIT/DC on the Actions page. The problem with using Bonus > Melee Attack is that it does it to *all* your melee weapons.
I certainly appreciate the response, but there is an issue with this method. It changes the hit roll of *all* my melee weapons. I only want it to affect this individual weapon. It does work with the attunement how I want, however. Thank you for the idea!

It shows as:
HIT: 9, 1d12 + 5 (STR mod), -2 Slashing, +3 Radiant.
After Attunement:
HIT: 11, 1d12 + 7, -2 Slashing, +3 Radiant.
Note: the -2 Slashing does NOT affect my other Slashing weapons the way Melee Weapons Attacks affected them.
Also, as a Bonus Action, this weapon can be changed from Slashing to Bludgeoning.
