In a floating pyramid hovering above a PC's castle, there is an internal maze of dungeon crawls where creatures struck by an artifact sword are imprisoned. Any PC's who attempt to explore there realize a number of things:
Teleportation and Planar magic does not work within the pyramid. Furthermore, items like bags of holding cease to function temporarily.
Rooms are either populated by a monster/creature or a trap.
Monsters struck by the artifact sword are trapped in rooms for eternity.
Only divine level magic or a special ritual in the center of the Pyramid may free them.
Monsters are not able to see or pass through doorways
Shifting rooms!
Whenever the PCs open wizard marked doors they can hear grinding stone, all rooms and corridors move around except for the room/corridor they are leaving and the room/corridor they are entering. THIS effectively makes them LOST within the pyramid prison.
There are only two true exits from the pyramid, one is a physical hole at the base of the pyramid. The other is a central chamber with the only working teleportation magic within the pyramid prison. The dungeon crawl is generated with an online dungeon generator and reset with every trigger (wizardmared door or artifact sword use). I designate the two exits for myself (DM) with every triggered 'reset'. It's proven to be a hard find for PCs inside.

When a party with no healer is just destroying your monsters, introduce a monster that translates all damage into health, and can only be harmed by healing magic.

A disease which immediately raises the character to level 20, but kills them over the course of several days. The player retains control of their character, except when they encounter someone they have more than a neutral reputation with, in which case the DM takes control and forces the character to kill the other person. *Note: includes other diseased party members.

The party is moored on a small, deserted island after a failed mutiny, where they have to slowly learn or figure out the secret that this island is the island that the (insert games pantheon of gods or other equivalent) meet every year to discuss... things. The thing is, the (insert) also throw vicious parties that other powerful deities attend, and if the party is cautious enough to try and overhear something 'accidentally' reveal somehow that if some entity who was not welcome on the island was discovered, they'd instantly be destroyed, and guess who isn't on that list? So it's a survival game of cat and mouse, when the cat isn't even aware of the mouse.... yet. >:)
