Creating rules for running monsters characters as PCs that actively punish anyone who tries to do so.

Volo's Guide has nothing on the 3rd Edition rules. I don't think that his blog is available anymore, but the guy who was in charge of writing them outright stated that he hated the idea of monstrous player characters and intentionally wrote the rules to make them unplayable.
Monster races unplayable? Yuan-ti Pureblood would beg to differ.

Bad Idea: gaining levels requires finding a trainer in your class, then paying them gold equal to the amount of experience it takes to get to that level.

You should have seen an Ogrillon Monk. It was ridiculous.
Instead of having spell slots that limit how many spells you can cast per day, spellcasting is based on making an Arcana (if arcane spellcaster) or Religion (if divine spellcaster) check against a DC of 15+the level of the spell you want to cast. Failure results in negative consequences that depend on how badly you failed the check. This means that you can theoretically cast Meteor Swarm at 1st level, but you're more likely to accidentally turn yourself into a tuna sandwich.
The spell failure rule reminds me of "Retrocasting". A rare few casters are gifted with a condition that can be a curse or a blessing in the right or wrong hands.

The cast a spell, one must spell the spell. Good for not confusing geas with geese, but good luck with prestidigitation.
