Do ghosts have to have a horrifying visage effect? I'm planning on using one as an NPC, but I don't want my players to turn into wrinkles, especially because this ghost will allow them to rest in the chamber that they are in and none of them have greater restoration.

You’re the DM. If you say that this ghost doesn’t have a Horrifying Visage then it does not. You could also say it has the option to turn it on/off. You could even say that every Ghost in your campaign world has that option.

Yeah, I definitely think the horrifying visage should be deactivate.
How else can you do the Hitchhiking Ghost story? Or any of the jump scares in every single ghost type movie? You should definitely think "hey, that person in the dim light is acting strange" just before they turn the HORROR on.

You could also say that not every ghost is the Ghost creature listed in the rules. Sometimes a dead guy is just a dead guy.
