I really like the new exhaustion rules, but I think a that they need something else to really make it scary. If I were to use the new exhaustion rules in my games, would this addition be too much?
For every 2 levels of Exhaustion, your movement speed is reduced by 5 feet, in addition to the normal penalties.
I agree that the lowering of speed should be included in exhaustion. However, I think the speed lowering should be more dramatic, possibly by lowering the speed by a third or 10 feet per two levels.

I chose every 2 levels because it rounds nicely with the standard 30-foot movement (meaning that at 10 levels of exhaustion you'd have 5 feet of movement).

I'm unsure about limiting character vision. The rules are already kind of loose on vision distance, and I don't see (no pun intended) how or why flavour-wise an exhausted character's vision would be impaired?

I envision (pun intended) that exhaustion means you're less likely to pay attention, some folks when they're particularly tired get blurry vision too. Things can be more difficult to discern at a distance, that's how I see levels of exhaustion affecting vision range.
